iPad Survey

Please take a few moments to tell us about your experience borrowing an iPad.

You are invited to participate in a research project to study how iPads can be used in higher education. You are being asked to participate since you have borrowed an iPad from the library. This survey about your experiences with the iPad will take less than five minutes of your time to complete. Your participation will help us improve our services and provide valuable information about how people use iPads in higher education. You must be at least 18 years old to participate.

We will do our best to keep your information confidential. To help protect your confidentiality, the survey will not ask information that will personally identify you or your name. All information collected in this study will be kept completely confidential to the extent permitted by law. Your email address will be requested so that we can enter you into a random drawing for a prize. However, it will be stored separately from any data collected in this study.

Your participation is voluntary and there is no penalty if you do not wish to participate. You may stop the survey at any time or skip any questions you do not wish to answer.

By completing this survey, you are indicating that you at least 18 years old, have read this document, have had any questions answered, and voluntarily agree to take part in this research study. You may print a copy of this consent agreement for your records.
